W-9 Policy
The Internal Revenue Service requires that the County of Madison obtain Form W-9 from independent contractors, consultants and other self-employed workers that are hired by the County. Please click here to download the required W-9 Form.
Payment Policy
The County policy is to issue payment within 30 days of receipt of accurate invoice or receipt of goods or services whichever is later.
